In order to schedule a consultation, you will need to complete the following:
1.) Complete the Member Registration Form.
2.) Fax medical records with vitals (blood pressure, pulse, heart rate, etc.)
dated within the last year.
3.) Fax Photo ID - Driver's license or State ID.4.) Fax in a signed copy of the Controlled Substance Form.
5.) Fax in a completed Primary Care Physician Notice Agreement.
